What Is Home Care Package

Home Care Package programme is a government-funded program that supports ageing Australians to live independently and safely at their own homes by providing a variety of long-term support and services.

There are four Home Care Package Levels:

Level 1

Basic care needs

Level 2

Low care needs

Level 3

Intermediate care needs

Level 4

High care needs

Case Management

Manage Your Home Care Package

P & J Care will arrange services for you based on your needs and preferences.

Our Case Management fee is lower than other providers because we want you to make the most out of your Home Care Package by spending the funds on care and services that you need.

Each client will be assigned to an experienced Case Manager who will design and manage your Home Care Package. This includs:   

  • Understanding your needs, goals and preferences
  • Organising and setting up services as well as ensuring that you are receiving appropriate care at all times
  • Monitoring your progress and responding to changing needs
  • Providing advice about options and implementing strategies
  • Undertaking assessments, developing care plans and budget plans

Our Home Care Package Services

Personal Care

Assistance with showering, dressing, grooming and continence management.

Domestic Assistance

Help with cleaning, meal preparation, and laundry.

Transport

Transportation for social outings, shopping, and appointments.

IT Support

Basic IT support to keep you connected to your family and friends.

Domestic Assistance – Change to Home and Garden

Help with cleaning, gardening, cooking and meal preparation. Home modifications to keep you safe and basic home maintenance.

Social Support

Social support visit for companionship in your home and connecting you with community groups.

Respite Care

Giving your family member and carers a break for a few hours, overnight or extended periods.

Nursing

Provides clinical support for medication prompting or health conditions management.

Allied Health Services

Allied health professionals, who provide services to maintain and improve ones health and wellbeing.

Am I Eligible For Home Care Package

✔ Over 65 years of age (age 50 if you identify as an Aboriginal or Torres Strait Islander)
✔ Living at home
✔ Need support to continue living at home safely

Apply For
A Home Care Package

Step 1

Contact My Aged Care on 1800 200 422 to request an assessment.

Step 2

My Aged Care will contact you with a time and date for your assessment.

Step 3

After the assessment you will receive a letter from My Aged Care to confirm the outcome of your assessment.

Step 4

Once your Home Care Package is assigned, we will work with you to coordinate the services in accordance with your care needs.
